When signed this document will acknowledge my statement of my faith to be the same as, or in no way incompatible with*, the Covenantal Theology of the Reformed faith as set forth in the inspired and inerrant Scriptures of the Old and New Testaments in the original autographa;

I further acknowledge that I have read and am in accord with at least two (2) of the following (please circle two or more):

• the Westminster Confession of Faith and Catechisms
• the Belgic Confession
• the Helvitic Confession
• the Heidelberg Catechisms
• the “Five Points of Calvinism” (Canons of Dordt)

and I agree to notify my regional committee and the Board of I.T.E.M., Inc. in the event of any change of my beliefs or position in any regard or that otherwise may or does affect my acknowledgment and subscription hereunto;
